Easy to Complete Evidence Forms for VIEI Resource Centers

Below you will find links to easy to complete forms that can serve as evidenec for your students.  Typically, the student should submit one of the following evidences:

  1. A completed Vision Study Guide
  2. A completed Examination Answer Sheet
  3. An essay written by the student

However, we have found that submitting these documents can be cumbersome if they need to be scanned, simply because of their length.  Below you will find links to download (in Microsoft Word format) simple forms that are less than 5 pages long to serve as student evidence for the required Community Service Units.  These forms include:

  1. A Mentor's Report
  2. A Short Supplemental Assignment

Student Evidence

The Mentor's Report is designed to be quickly completed by the sponsoring pastor, Resource Center direcotr, or spiritual mentor for the student.  The Mentor's Report contains a series of checkboxes, asking the mentor to mark that the student has achieved competency through the course of study.  The Mentor's Report should be completed in four easy steps: 1) Print It, 2) Sign-It, 3) Scan It, 4) Submit It!

Student Evidence

We are required to have a signed mentor's report, so we ask that you fill out the document for each student, print it, sign-it, and either scan and submit, fax, or mail it to Vision.

The Short Supplemental Assignment: Completed by Students

The Supplemental Assignment is to be completed by the student.  We are aksing for brief responses to each area of competency (a short paragraph or 2-3 sentences).  The Assignment simply asks the studnet who he or she is serving in the local church and community and how that service has led to competency when coupled with their studies with Vision International.  This form can be submitted electronically by the student or printed and either scanned and submitted, faxed, or mailed to Vision.

Additional Evidence for the Advanced Diploma

Note that if you elect to use the Mentor forms and the Supplemental Assignments, you will need to submit 1 additional evidence for each student for each Community Service module on the Advanced Diploma level.  This should be the completed exam, essay, or study guide.
